HIPAA Notice & Communication Disclaimer
Floora Pelvic Floor Physical Therapy ("Floora PT") is committed to protecting the privacy of your health information. This notice summarizes how Protected Health Information ("PHI") may be used and disclosed, your rights under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), and the limitations of communicating with us through unsecured channels such as our website contact form, email, and standard text messages.

1. Important Communication Warning
The website contact form, general email, and standard SMS text messages are not encrypted or HIPAA-secure.
Please do not include any of the following in a contact-form submission, email, or text message:
- Detailed descriptions of medical symptoms or conditions
- Medical records, lab results, or imaging
- Insurance numbers, Social Security numbers, or other sensitive identifiers
- Any information you would not want to be intercepted or seen by an unauthorized party
If you need to share clinical information securely, please call us at (916) 756-3165. Once you become a patient, you'll also receive secure-messaging access through the Jane patient portal — our HIPAA-compliant electronic health record (EHR) system — delivered to you by email after booking.
2. Who This Notice Applies To
This notice applies to:
- Current patients of Floora PT
- Prospective patients considering becoming a patient
- Anyone communicating with us about a health-related matter
3. How We Use and Disclose Your Health Information
Floora PT may use and disclose your PHI for the following purposes without your additional written authorization, as permitted by HIPAA:
- Treatment — to provide, coordinate, or manage your physical therapy care, including consulting with other healthcare providers involved in your care.
- Payment — to bill and collect payment for services, verify insurance eligibility, and obtain pre-authorizations where applicable.
- Healthcare Operations — to support our internal business activities, such as quality assessment, scheduling, credentialing, and training.
We may also use or disclose your PHI:
- To remind you of appointments (by phone, voicemail, text, or email — with your consent)
- As required by law (e.g., for public health reporting, subpoenas, court orders)
- To business associates who perform services on our behalf and are bound by a written Business Associate Agreement
- With your written authorization for any other purpose
We will not sell your PHI, use it for marketing without your written authorization, or disclose psychotherapy notes without your authorization.
Our Business Associates
We work with the following HIPAA-compliant business associates, each of which operates under a signed Business Associate Agreement (BAA) with Floora PT:
- Jane (jane.app) — our electronic health record (EHR) and patient portal, where clinical notes, intake forms, treatment records, and secure patient communications are stored.
- Pelvibiz / GoHighLevel ("GHL") — our scheduling and patient-communication platform, which handles online booking, appointment confirmations, SMS reminders, and storage of basic patient contact and scheduling information.
4. Your Rights Under HIPAA
You have the following rights regarding your PHI:
- Right to access — request a copy of your medical records (in paper or electronic form, where available).
- Right to amend — request corrections to information you believe is inaccurate or incomplete.
- Right to an accounting of disclosures — request a list of certain disclosures we have made of your PHI in the prior six years.
- Right to request restrictions — ask us to limit how we use or disclose your PHI for treatment, payment, or operations. (We are not always required to agree, but we will consider every request.)
- Right to confidential communications — request that we contact you in a specific way (e.g., by mobile phone only, not at home) or at a specific location.
- Right to a paper copy of this notice and our full Notice of Privacy Practices, even if you have agreed to receive it electronically.
- Right to file a complaint — see Section 7 below.

5. Breach Notification
If a breach of your unsecured PHI occurs, we will notify you in writing as soon as possible and within the time frames required by HIPAA's Breach Notification Rule.
6. Authorization
Uses and disclosures of your PHI not described in this notice, or otherwise permitted by HIPAA, will be made only with your written authorization. You may revoke your authorization at any time, in writing, except to the extent we have already acted in reliance on it.
7. How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint:
With Floora PT — Contact our Privacy Officer (see Section 8). We will not retaliate against you for filing a complaint.

9. Changes to This Notice
We reserve the right to change this notice and our privacy practices at any time. Any changes will be effective for all PHI we maintain, including PHI created or received before the change. Updated notices will be posted on our website with a revised "Last Updated" date and made available at the clinic.
